COPPA PARENTAL CONSENT FORM

Notice to Parents: In compliance with the Children’s Online Privacy Protection Act (COPPA), parents (or legal guardians) of children under 13 years of age must consent to collections, uses, and disclosures of the personal information and personal work such as an essay or artwork of their children collected by Megadolls Inc. on Megadolls’s websites, or www.megadolls.com.

Megadolls’ COPPA statement is incorporated in the website Privacy Policy here: Privacy Policy. Megadolls may have collected your online contact information from your child and the name of the child or the parent to obtain your consent.

Your permission is required to collect, use, or disclose your child’s personal information. Unless you permit us, we will not grant your child access to any Megadolls website account or participate in any Megadolls event or offer, including participation or submission of their Artwork in the I AM contest.

Megadolls website accounts provide access to content, materials, and resources related to Megadolls’ programs and activities. These include information about the I AM program and the relationship between Megadolls and your child’s school through the School Referral program.
